Pharmacists in Primary Care

Primary Care Pharmacists are highly experienced and are able to assess, diagnose, treat and manage patients with medication issues. Practice Pharmacists have added extra support to the primary care team and strengthen the service we provide.

Primary Care Pharmacists are offered as an alternative to seeing your GP for Medication reviews. As well as supporting some chronic disease annual reviews such as Diabetes.  Our service aims to improve your access to the Right Care, at the Right Time, from the Right Clinician.

The benefits of the Primary Care Pharmacists

  • Typically will be seen quicker than a GP appointment
  • Faster and longer appointments to review your medication regime particularly following a change/ addition or removal of medication
  • Longer more in-depth appointments (20-30 minutes)

Service eligibility

  • Patients aged 18 or over
  • Medication reviews for conditions such as
    • Diabetes
    • Asthma
    • Heart Failure
    • Cancer
    • Arthritis
    • Pain medication

The service is inappropriate for you if you have any of the red flag conditions as well as your medication concern.

How do I book in?

Please contact your GP practice as normal and request an appointment from the Receptionist. Clinics will be spread across all 3 practices and patients will have access to all days and times available. This  may mean your appointment could be at a different site to your regular practice.  Initially appointments will be by telephone and if the clinician feels a face to face appointment is required, they will discuss this and book you in.

How long will my appointment be?

20-40 minutes dependent on how many medications you are taking
