The role of a Mental Health Advanced Clinical Practitioner, is to predominantly support those with acute and complex presentations in primary care and work with patients to risk assess and signpost as well as review medication and initiate where this has been clinically identified. They are accessible via GP/Duty referral, and an appointment can be made with them via this route if deemed suitable. They are able to assess and refer to other suitable services where this is indicated. They are able to provide psychoeducation around self management to support patients in managing independently in the community.
This role within the PCN offers patients an enhanced and specialist practitioner for mental health needs and ensures that timely and effective advice and guidance is given in relation to treatment options for mental health difficulties. The PCN is passionate about ensuring there is access to meaningful mental health support for the patient population.
